The Community Health Clinics of Northeast Texas are here to serve you! Last year, the Community Health Clinics of Northeast Texas provided over 23,000 health and dental visits to residents of Tyler, Smith County, and adjacent communities. We are the premier provider for Medicaid, Medicare, S-CHIP and uninsured patients in the community.

The Medication Assistance Program at the Primary Care Clinic provides to patients approximately $1 million worth of free medications per year, and over 800 referrals are made annually to the PATH prescription program.

Our services include: well-women check-ups, preventive healthcare for men and women, physicals, children’s healthcare, children’s dental care, well-child check-ups, education and testing for sexually transmitted diseases (STDs), mental health evaluation and counseling, healthy lifestyle education, health screenings for diabetes, HIV, high blood pressure, and other chronic conditions.
